Il Nuovo Postiglione - Why Pregnancy Cost Estimates Fail: A Family’s $9,000 Surprise

Grace Griffin lay awake at night worrying about the financial aftermath of welcoming her first child. She and her husband, Daniel, both 26, had attempted to budget for their daughter’s arrival by consulting a cost estimator linked to Grace’s UnitedHealthcare student plan. The tool, however, demanded specific medical billing codes for procedures she was likely to receive—information that was unavailable before conception.

Undeterred, Griffin searched the insurer’s website for a Summary of Benefits and Coverage document. It estimated out-of-pocket costs for a routine pregnancy and delivery at approximately $2,700. The couple believed this figure was manageable and began saving accordingly. They assumed they could cover the predicted expenses without derailing their finances.

As the pregnancy progressed, those assumptions unraveled. Early in the gestation, Grace met her $500 deductible, the threshold she had to pay before insurance coverage activated. Her plan also required a 20% coinsurance for in-network care. This meant that for every $100 billed for services such as routine blood work, she was responsible for $20.

The financial burden accelerated during diagnostic testing. Routine blood tests to monitor iron levels and screen for gestational diabetes cost several hundred dollars per visit. "When I started getting lab work is when we started realizing expenses would add up quickly," Griffin said.

In June, after what was described as a routine pregnancy and delivery, the couple welcomed their daughter. Shortly thereafter, they received an explanation of benefits notice from the hospital. The document revealed that the facility had charged roughly $18,000 for the delivery. The notice indicated that the family was liable for a significant portion of that total.

"I thought, ‘Oh no, it’s going to be a lot,’" Griffin recalled. "Much more than we budgeted for."

By early October, the Griffins’ out-of-pocket expenses had surpassed $9,000. This amount was more than three times the general estimate published by their insurer. The discrepancy underscored a broader issue in healthcare transparency: the difficulty patients face in predicting actual costs despite regulatory efforts to improve visibility.

For years, federal lawmakers have sought to address this opacity. Sabrina Corlette, a research professor and co-director of Georgetown University’s Center on Health Insurance Reforms, noted that Congress mandated insurers to provide consumers with an individualized "advanced explanation of benefits." This document is intended to estimate what scheduled care will cost the patient.

The requirement was part of the 2020 No Surprises Act. The rule was originally scheduled to take effect on January 1, 2022. However, federal regulators have not yet enforced the mandate, despite persistent pressure from members of Congress. "So, it’s basically 4½ years since this was supposed to be available, and I think it’s a source of a lot of frustration for a lot of folks," Corlette said. "People have a right under the law to know these things."

Another regulatory shift is approaching. Beginning January 1, long-standing maternity billing codes that bundle much of pregnancy care will be replaced with more detailed coding for prenatal care, labor, delivery, and postpartum care. Corlette suggested this change could make it easier to identify the cost of individual services. However, she expressed uncertainty about whether it would help patients stick to a budget.

"I could see where it might be advantageous to the provider," Corlette said. "But I’m not sure I’m seeing benefits to the patient."

Until out-of-pocket maternity expenses become easier to estimate accurately, health experts recommend five specific strategies for families preparing for birth costs.

First, enroll your newborn immediately. Birth triggers a special enrollment opportunity, allowing parents to add a baby to a health plan outside the annual open enrollment period. Many employer health plans require parents to request enrollment within 30 days of birth. Marketplace plans, which are purchased individually rather than through an employer or government program, generally allow 60 days. Missing these deadlines can leave a newborn uninsured until the following year.

Michelle Critzer of Ventura, California, experienced this risk firsthand. A sales representative and mother to a toddler and a newborn, Critzer missed the enrollment deadline for her second son, Hudson, who was born in February 2026. She has since asked her insurer to make an exception without success. "The way they set hard must-dos within 30 days of having a baby is actually insanity," she said.

Critzer’s family eventually enrolled Hudson in a health plan through her husband’s workplace, which offered a 60-day window. However, Critzer noted that this coverage was less robust than her own insurance. She remains grateful for the coverage but continues to press her own plan to enroll him formally.

Second, understand key financial metrics, particularly the maximum out-of-pocket (MOOP). Budgeting begins with identifying the deductible—the amount paid before significant coverage kicks in—and copayments, which are fixed fees per service. Coinsurance is another variable; it requires the patient to pay a percentage of each service’s cost.

The MOOP represents the annual ceiling on what a patient pays for covered, in-network services. Under the Affordable Care Act, the 2026 MOOP cap is $10,600 for an individual and $21,200 for a family. Once spending reaches this limit, insurance covers 100% of further in-network costs for the year. Some plans may set lower maximums.

A critical complication arises if a pregnancy spans two plan years. The MOOP resets to zero at the start of the new year, potentially forcing families to max out their expenses in consecutive years. Additionally, some plans maintain separate MOOPs for in-network and out-of-network care. Out-of-network limits can be double the in-network limit, providing a strong incentive to verify that all providers are in-network.

For the Griffins, the family MOOP was $18,000, while Grace’s individual maximum was $9,000. Their expenses approached these caps rapidly.

Third, prioritize in-network care. Selecting an in-network obstetrician does not guarantee that all members of the care team are in-network. Patients must verify the status of hospitals, laboratories, and imaging facilities used by their doctor’s office. Corlette advised patients to explicitly instruct their doctor’s office to send tests to in-network providers.

Federal law protects patients who receive care at an in-network hospital or facility from out-of-network "surprise bills" issued by out-of-network providers. However, verification remains the patient’s responsibility before services are rendered.

Fourth, scrutinize every bill and file appeals when necessary. The Affordable Care Act requires certain recommended prenatal services to be covered without additional cost. These include prenatal visits, folic acid supplements, anxiety and depression screening, and breastfeeding and lactation counseling and supplies. Despite these mandates, costs are sometimes mistakenly passed on to patients.

Corlette suggested that patients become familiar with these covered services and check every bill for accuracy. If an insurer refuses to pay for a covered charge, filing an appeal is a necessary step to correct the error.

Fifth, seek financial assistance early. Most hospitals offer financial assistance programs designed to reduce bills or provide manageable payment plans. Doctors and other providers may also offer assistance if asked. Corlette emphasized that any arrangement must be formally agreed upon in advance to prevent accounts from being unexpectedly turned over to collections.

Grace Griffin qualified for Michigan’s Rx Kids program, which provides eligible families in certain communities with $1,500 during pregnancy and $500 a month for the baby’s first six months or year of life. This support helped preserve the family’s emergency savings.

"It takes a lot of pressure off," Griffin said. Her experience serves as a cautionary tale for parents navigating the complex intersection of healthcare policy, insurance mechanics, and personal finance.
